PHYSICALS REQUIRED FOR HS Players
DON’T FORGET!
All players competing at the High School level are required to complete the official PIAA physical and obtain a physician’s signature certifying the player is ready and able to compete.
A COPY of the completed PIAA is a required part of the Haven Hockey Registration packet for
